Please print, and complete the forms linked below prior to coming to camp. Please bring these completed forms with you to camp. Your child will not be able to attend camp without a completed Health Exam.

Children with Type 1 Diabetes who are not yet eligible for our overnight camp, or those who just want to check us out are invited to attend our Day Camp. All attendees must be chaperoned.
